HOURS OF OPERATION

Hours of operation are 9.00am – 5.00pm (Monday to Friday). Other planned activities may take place outside of these hours.

  Community Mental Health

The Community Mental Health Team comprises mental health professionals and support staff including.

  • Psychological Therapists

CLIENT GROUP

Adults aged between 18-64 years experiencing mental health problems. Service users known to the service who turn 65 years will remain with the adult service, however, care maybe transferred to Psychiatry of Old Age Service if indicated.

REFERRAL PROCESS

Referrals can be made by:-

General Practitioner

  • Other Community Mental Health Teams
  • Other Specialist Mental Health Services
  • Forensic Medical Officer
 

AIMS OF THE COMMUNITY MENTAL HEALTH TEAM

  • At all times provide a quality service
  • Provide a Psychosocial assessment to those who present with features of a mental illness.
  • To sign post to other appropriate services.
  • Where appropriate, provide treatment and management within available resources.
  • To promote service user and carer involvement in treatment plans.
  • To work in partnership with relevant agencies for the benefit of service users and carers.

FUNCTIONS OF THE COMMUNITY MENTAL HEALTH TEAM

  • Provide a Multi-disciplinary mental health service to the local area population.
  • Each discipline will utilise their skills and knowledge specific to patient need.
  • To provide services which are accessible to the local community.
  • Raise awareness of mental health issues within the community.
  • Work in accordance with legislation, trust policies and procedures.
  • Make best use of available resources.
  • Provide education and support to carers and families.
  • Provide services which are, equitable, comprehensive and of high quality.
  • Multi-disciplinary team approach.
  • Providing evidence based therapeutic interventions.
