The Mental Health Service will help provide a Psychosocial assessment to those who present with features of a mental illness.

Post referral, if appropriate they will signpost patients to other appropriate services or organisations that may be of help.

The Community Mental Health Team aims to deliver a quality service through various functions.

Aims of the Community Mental Health Team

  • At all times provide a quality service
  • Provide a Psychosocial assessment to those who present with features of a mental illness.
  • To sign post to other appropriate services.
  • Where appropriate, provide treatment and management within available resources.
  • To promote service user and carer involvement in treatment plans.
  • To work in partnership with relevant agencies for the benefit of service users and carers.

Functions of the Community Mental Health Team

  • Provide a Multi-disciplinary mental health service to the local area population.
  • Each discipline will utilise their skills and knowledge specific to patient need.
  • To provide services which are accessible to the local community.
  • Raise awareness of mental health issues within the community.
  • Work in compliance with legislation, trust policies and procedures.
  • Make best use of available resources.
  • Provide education and support to carers and families.
  • Provide services which are, equitable, comprehensive and of high quality.
  • Providing evidence based therapeutic interventions.
